Green Prescription - For Health Professionals

Green Prescription and Active Families are both free behaviour change intervention programmes. A referral can be made by a registered health professional, or a patient can complete a self-referral. Staff provide free individual or whānau focused support to enhance physical, mental, and social wellbeing through physical activity and healthy eating support. A patient must be medically stable to be referred and will be supported one-on-one for up to three months with access to any of our exercise groups that we have running (please check the Green Prescription tab for further details on any groups running).


​​​​​​​Active Families Referral Criteria

Whānau with tamariki and rangatahi aged 4 to 17 who would benefit from healthy lifestyle support in physical activity and nutrition.

Not meeting Ministry of Health physical activity guidelines for children: being active 60 minutes daily at vigorous intensity (rosy cheeks).

Not meeting Ministry of Health eating guidelines.


Green Prescription Referral Criteria

Adults aged 17+ who would benefit from healthy lifestyle support in physical activity and nutrition.

Not meeting Ministry of Health physical activity guidelines for adults: being active 5 times a week for 30 minutes at moderate intensity (TALK test).

Not meeting Ministry of Health eating guidelines.
