FOR HEALTH PROFESSIONALS
Green Prescription and Active Families are both free behaviour change intervention programmes. A referral can be made by a registered health professional, or a patient can complete a self-referral. Staff provide free individual or whānau focused support to enhance physical, mental, and social wellbeing through physical activity and healthy eating support. A patient must be medically stable to be referred and will be supported for up to six months.
Active Families Referral Criteria
Whānau with tamariki and rangatahi aged 4 to 17 who would benefit from healthy lifestyle support in physical activity and nutrition.
Not meeting Ministry of Health physical activity guidelines for children: being active 60 minutes daily at vigorous intensity (rosy cheeks).
Not meeting Ministry of Health eating guidelines.
Green Prescription Referral Criteria
Adults aged 17+ who would benefit from healthy lifestyle support in physical activity and nutrition.
Not meeting Ministry of Health physical activity guidelines for adults: being active 5 times a week for 30 minutes at moderate intensity (TALK test).
Not meeting Ministry of Health eating guidelines.
